Immunizations aren't just for preschoolers. Teens
should receive the following immunizations before they graduate from high
school:

Meningococcal Meningitis

Influenza

Tetanus-Diptheria-Pertussis (Tdap)

Varicella (chickenpox, if needed)
Call or visit your local Super Shot clinic. Vaccinate before you graduate!

Physicians Health Plan, a long-time community partner of Super Shot, Inc., is
committed to helping prevent childhood diseases. This valuable local
organization provides immunizations for children in Allen County at 29 clinics
a month. Super Shot charges a minimal fee of $8 per shot. Medicaid is accepted
and no one will be turned away for inability to pay.
Super Shot is not a government program, it's funded by our community
and your donations are critical to its continued success. Please help protect
