PHP has always recognized the importance of maintaining the confidentiality of health information. In response to HIPAA ’s Privacy Rule, PHP’s privacy practices have been published and distributed since April 2003. This document summarizes how protected health information may be used and disclosed at PHP and defines specific rights each individual has regarding their protected health information.
