Focus on Health
Physicians Health Plan has pledged to play a leadership role in the community by
supporting programs that promote health awareness and inspire healthy lifestyle
changes. For that reason, we have been pleased to donate funds to the Focus on
Health program for several years.
It is our wish that this program will continue to benefit our community as it
has in years past. In hopes of its success, and the success of the community's
healthy lifestyle goals, we will provide Focus on Health community information
on our Web site as it becomes available.

Free Immunizations With Super Shot!
  Immunization is an important aspect of maintaining
good health. It goes beyond benefiting our children; it benefits the entire
community by preventing childhood diseases. That is why PHP is committed to
this endeavor and proudly sponsors the Super Shot, Inc. program. This program
provides free immunizations for children twenty-four days per month at
twenty-four different locations.

"Here is an insurance company that pays for immunizations sponsoring free shots
for people. It's amazing" stated Kelly Zachrich, Executive Director of Super
Shot. PHP has never wavered in our support of Super Shot by providing almost
50% of their funding as well as employee volunteers. PHP President and CEO Jay
Gilbert states, "We believe Super Shot plays an important role in preventing
illness by educating and providing immunizations to individuals that may not
otherwise have access to preventive healthcare. This program benefits so many
and being a part of it is important to us. Wellness and meeting the healthcare
needs of the community is what Physicians Health Plan is all about."

continuing efforts in the education and distribution of immunizations for
children in our community.

Matthew 25
In 1992, through Physicians Health Plan, local doctors created an annual trust
fund to support non-profit health-related organizations. The Plan donates
$300,000 annually to serve low income, high risk, medically underserved, and
elderly persons in northern Indiana.
Matthew 25 Health and Dental Clinic is one of the organizations that receives
funding through PHP's Physician Trust Fund. For over 25 years, Matthew 25 has
served the medical, dental, and vision care needs of people who are unable to
access healthcare through traditional means. Congratulations on a job well
done and thank you for working to improve the quality of life in our community!

Safety Village... "If You Involve Me, I Will Learn"
 Safety Village/Survive Alive House consists of thirty
miniature buildings and a downtown area with sixteen structures. This village,
which is located at 7800 S. Anthony Blvd. behind Southtown Mall, provides
police and fire educational curriculums free of charge to all area schools,
public and private. In addition, summer programs are available and families may
visit an open house once a month or by appointment. The Safety Village's sole
purpose is "to teach fire and police safety to elementary students so that they
might live to become adults."
Physicians Health Plan is the proud sponsor of a Safety Village building. We
believe that the education of safety to the children in our community is
crucial in the health and wellbeing of our community as a whole, now and in the
future.
