If you are a PHP member residing outside of PHP's participating provider service area, you may have access to an alternate healthcare provider network. The network logo will be displayed on your traditional member ID card in the front lower left corner as: CCN, Beechstreet, IHN, HPO, etc.

You will find a coordinating Provider Directory Access Code on the back of the card. When prompted, enter the Provider Directory Access Code and follow the on screen instructions. The healthcare provider information that you will receive is specific to you and your local network.
