Am I required to use a PHP participating network Dentist for coverage?

If your PHP member ID card states on the front lower left-hand side that your coverage is provided by PHP Insurance Company of Indiana, Inc., you are not required to use a network dentist to receive coverage.  You may receive services from any licensed dentist under your benefit plan.
