PHP offers the following weight loss incentive programs:

Inches Away - For Adults

PHP's Inches Away program is designed to encourage, support, and reward your goal of acheiving a healthy weight. Inches Away reimburses portions of the cost of a physician-supervised weight loss program, over an 18-month period of continued treatment and successful completion of a weight loss program. Inches Away - For Adolescents

The adolescent program reimburses a total cost of a weight loss program, and is paid in one payment after completion of the program. Parkview SHAPEDOWN and the Lutheran ACE program qualify for this incentive program. Click here to download the Inches Away brochure.

PHP Weight Loss Reimbursement Program

PHP also offers an independent incentive program, with a lifetime maximum reimbursement of $200. To qualify, the following must be documented in your PHP physician's records:

• Your weight prior to beginning the program.
• Physician's recommended weight loss goal.
• Maintained recommended weight loss for the duration of one year.

  Your Responsibilty:

• Document all costs incurred while attaining the recommended weight. (This includes all costs associated with the weight loss program you choose to attend such as lab fees, office visits, supplements, etc.)
• Maintain weight loss for the duration of one year.
• Maintain active coverage with PHP throughout the entire process.
• Submit all paid receipts and a copy of the pertinent documented medical records to:

PHP Smoking Cessation Program

In effort to support healthy lifestyles, PHP has established two separate programs to help members who want to quit their smoking habit. Select the program that best suits you!

The CHANTIX™ Benefit Program

CHANTIX™ is a covered medication specifically developed to help individuals kick the tobacco habit and available under your benefit plan. PHP will cover up to two consecutive twelve-week courses of CHANTIX™ in a calendar year. Each twelve-week course consists of three individual prescriptions, each with its own 2nd tier co-payment or co-insurance. The success rate to quit smoking is higher if an individual takes advantage of a support program in conjunction with drug therapy. A program called "GetQuit" is free of charge to those using CHANTIX™. Logon to www.chantix.com for more information.

Smoking Cessation Reimbursement Program

A reimbursement program is available for members who elect not to use CHANTIX™ medication to quit smoking. PHP will reimburse a member up to $300 for charges paid for any type of therapy (including nicotine patch, nicotine gum, nicotine inhalers, and/or classes) if the member remains smoke-free for at least one year after the end of the therapy. Here are more program details:

• The PHP member is responsible for all smoking cessation costs while becoming smoke-free.
• After a one year, smoke-free period, the PHP member must have a PHP participating physician complete and submit the Non-Smoking Verification Form.
• The PHP member must submit all paid receipts to PHP for consideration. After review, PHP will reimburse the member up to a maximum of $300 (per lifetime). Depending on the therapy used, copays may apply.
• The member must be an active PHP member throughout the entire process.
• Return the Non-Smoking Verification Form and paid receipts to:

Sweet PHP Rewards

Sweet PHP Rewards is a Diabetes Management program aimed at assisting those members with a diabetes diagnosis to make healthy lifestyle changes. The focus is on educating members about healthy lifestyle including diet, exercise, taking daily medications, and frequent blood testing.

Sweet PHP Rewards members receive monthly incentives such as reduced or eliminated copays and deductibles on medication and testing strips. This program consists of one-on-one, phone-based assessment and personalized coaching to encourage behavioral lifestyle changes. Click here for a program brochure.
