EyeMed Vision Care is a point-of-service discount-only program that
provides you and your dependents with significant savings on eyewear
needs including frames, lenses, and contacts.
How does the EyeMed discount plan work?
When choosing a service location in the EyeMed Vision Care Network:

There will be no claim forms to fill out and no waiting for reimbursement.

To have full advantage of your discount benefit, present your EyeMed identification
card at one of the many participating vision care locations.

EyeMed discounts will be automatically calculated.

Your EyeMed discount benefit may be used as often as you like, with no
limit to your savings.
Am I eligible?
While part of PHP, you and your dependents are eligible to receive discounts under
EyeMed. An EyeMed ID card is automatically mailed to your home upon enrollment in
your employer based health plan. Just present your EyeMed ID card at any
of the participating vision care centers prior to making a purchase, and
receive up to 40% OFF the retail cost of eyewear and contact lenses.

LASIK and PRK Benefit
All PHP EyeMed Vision Care members are provided with a laser vision correction
discount. EyeMed and LCA-Vision have arranged to provide this benefit through
the U.S. Laser Network. You are entitled to a 15% discount on the usual and
customary fees for LASIK and PRK procedures, or a 5% discount on any promotional
pricing, whichever is the greater benefit. One easy phone call to 1-877-5LASER6
begins the process of using your discount. No claim forms are needed, making it
a hassle free process.
Contact Lens By Mail Program
You may order replacement contact lenses for great prices via the internet, and
have the contacts mailed directly to your home. The service is for replacement
contact lenses only, and your EyeMed discount will not apply to the service. Your
initial pair of contact lenses must still be purchased from your eyecare provider
to ensure proper fit and follow-up care. Simply visit
