HMO
An HMO is a copayment-based, comprehensive healthcare benefit plan where
Physicians Health Plan has contracted with a wide range of doctors, hospitals,
and other healthcare professionals to form a network of healthcare providers.
Within this network, members have open access to over 2,500 providers which
means members choose the healthcare provider they want to see, from primary
care to specialists, without any special arrangements through the plan.

Point of Service
A Point of Service (POS) product combines an HMO benefit package with the
out-of-network benefit coverage. This allows members to choose healthcare
services from a non-participating provider at a reduced benefit level.

Dental Programs
Dental care plays a vital role in an individual's overall level of wellness and

Coverage is available as a stand-alone product or partnered with our medical
coverage. Benefit plans can be structured in one of two ways - using a dental

Group Life and AD&D
Life insurance offers benefit coverage provided in the form of financial income
to a named beneficiary in case of death. Short Term Disability and Long Term
Disability insurance is also available. Disability insurance is designed to
help replace lost income should one become totally disabled and unable to work
due to a non-occupational accident or sickness.
