How do I become a participating provider?
PHP requires that each practitioner complete a Participating Application Request Form. The form can be faxed or mailed to our Credentialing Coordinator at (260) 432-0493 or Physicians Health Plan, ATTN: Credentialing Coordinator, 8101 West Jefferson Blvd, Fort Wayne, IN 46804. What types of providers are credentialed and contracted for participation in the Network?
We currently credential and contract for the following providers: MD, DO, OD, DPM, DDS, DMD, PA, CRNA, CNM, and NP.
What other providers are considered for participation?
We accept Letters of Interest and Provider Application Requests for PhD, PsyD, LMFT, LCSW, LMHC, DME, and HHC.
How do I update any changes to my practice information on file with PHP?
To update information regarding your practice, please provide a written explanation and the effective date of the change. Fax your notification to Provider Network Services at 260-432-0493.
What is CAQH and how can I find out more about this organization?
CAQH is a non-profit alliance of networks, trade associations, health plans, and networks that collaboratively initiated a Universal Credentialing Data Source to streamline credentialing processes. This process has eliminated the need for multiple credentialing applications. Please visit their website for additional information at www.caqh.org.
How do I obtain my NPI Provider Number and what do you require once I receive it?
NPI is a National Practitioner Identifier. For more information, visit www.cms.hhs.gov/NationalProvIdentStand/. We will accept individual and group NPI numbers depending on how you submit your claims. All EDI claim submissions require you to use the appropriate NPI number.
