How to File a Grievance

At PHP, we want to provide you with the highest quality of healthcare coverage. If you have concerns about your coverage, or questions regarding a claim payment, a grievance procedure has been established that allows you to request a reconsideration of any decision that you disagree with. To begin the process, please call or write to your Customer Service Department, stating your question or concern. We will assist you in any way we can.

For questions about your rights or for assistance, you can also contact:  the Employee Benefits Security Administration at 1-866-444-EBSA (3272) if your coverage is a group health plan through your employer and/or if coverage is insured (group or individual) you may contact your State's Department of Insurance.
